Tip: Improving Note/Letter Quality in i-scribe by Clarifying Details During Consults

Clarifying details (especially for the Diagnoses, Examination, and Plan sections) can significantly improve your note and letter quality. There are some tips below.

To get the most accurate and clinically useful notes from i-scribe, it’s helpful to actively clarify details while transcribing. Small verbal clarifications can significantly improve the quality, structure, and accuracy of generated documentation.

Why This Matters

i-scribe relies on what is said during the consult. If details are implied (e.g. gestures, assumptions, or shorthand), they may be:

  • Missed entirely

  • Interpreted incorrectly

  • Structured in ways that don’t match your preferences

By clearly stating key details, you ensure:

  • Higher clinical accuracy

  • Better structured notes

  • Reduced need for editing afterward

Key tips:

1. Set Expectations with the Patient

We recognise this won't work for every clinician. However, after you have properly consented the patient for i-scribe, you can also say:

“Since my computer is writing my notes for me, you’ll hear me say some details out loud. Please let me know if you have any questions about that."

This:

  • Maintains transparency

  • Keeps the patient comfortable

  • Allows you to freely clarify details throughout the consult

After having said the above phrase it allows you to more comfortably follow the tips below.

An alternative option is to wait until the patient has left the room and then clarify details (especially for the Diagnoses, Examination, and Plan sections).


2. Clarify Implicit Information

Patients often communicate information non-verbally (e.g. pointing or gesturing). These details need to be made explicit for i-scribe to understand the information.

Example:

  • Patient gestures to knee

    • You could say: “Just to clarify, that is the right knee." 

This ensures the note correctly captures laterality and avoids ambiguity.


3. Guide the Diagnosis, Examination, & Plan Sections

If left entirely to an AI system, diagnosis and plan sections may be:

  • Ordered differently than you prefer

  • Worded in a less structured way

  • Make assumptions that are incorrect (due to lack of explicit information)

To improve this, summarise explicitly out loud. This can often work best at the end of the consult. For example:

Just to summarise, your diagnoses are:

  1. Osteoarthritis of the right knee

  2. Hypertension

  3. Type 2 diabetes”

This helps i-scribe:

  • Structure lists clearly

  • Preserve your preferred wording and diagnoses

  • Capture clinical priorities accurately

This approach also works well with the Plan section.
